Patient Responsibilities

Appointments and Cancellations
Appointment Scheduling
- Appointments can be scheduled by phone, email, or through our website
- We strive to accommodate your preferred appointment time

- Please arrive 10 minutes before your scheduled appointment
Cancellation Policy
24-Hour Notice Required: Please provide at least 24 hours notice for appointment cancellations or rescheduling.

Late Arrivals
- Patients arriving more than 15 minutes late may need to be rescheduled

Payment Terms and Insurance

- Payment is due at the time of service unless prior arrangements have been made

Insurance

- Future appointments may be declined if account is past due

While we strive for excellent results, we cannot guarantee specific outcomes because dental treatment results can vary based on individual factors including:

Limitation of Liability

- We are not liable for delays or inability to provide services due to circumstances beyond our control (emergencies, natural disasters, equipment failure, etc.)
- Our liability is limited to the cost of the specific service in question
Note: Nothing in these Terms excludes or limits our liability for death or personal injury caused by negligence, fraud, or other liabilities that cannot be excluded by law.

Important: Our clinic is not an emergency medical facility. For life-threatening emergencies:

Examples of dental emergencies: severe bleeding, facial swelling affecting breathing, trauma with tooth loss, severe pain uncontrolled by medication.
